Mission of Clinical Case Management

The mission of Clinical Case Management is to provide students with support in accessing community resources so that they are better able to focus on their personal, academic 职业目标. 

Clinical Case Management Services

Clinical Case Management helps students to access resources in the community to assist with a variety of needs including legal services, ongoing counseling, health insurance, childcare, addiction-related counseling, support groups, m一个y-management resources, dental/vision care, disabilities resources, eating disorder treatment/resources, immigration resources, immune disorder testing/services, information about public benefit programs, intimate partner violence resources and veterans resources.

期待什么

Clinical case management services are time-limited to assist students in obtaining support in connecting with on-campus and community resources.  Clinical Case Management appointments take place in the 学生健康中心, third floor in room 300B. There is no additional charge for the services. Students are eligible for brief clinical case management sessions per semester of case management services in addition to personal 咨询预约.

During your first meeting with the clinical case manager, you will discuss your situation and identify what types of community resources may be available. 临床病例 manager can help you contact these resources, set goals, and plan for next steps. Some students find that they only need 一个 appointment to meet their needs; others may decide with the clinical case manager to return for an additional appointment.

How to 预约

  • If you are not currently seeing a CAPS personal counselor, please schedule an initial case management  appointment by calling 408-924-5678 or by coming to our location (学生健康中心, third floor, Room 300B).
    If you are currently meeting with a CAPS personal counselor, please let your counselor know that you would like to meet with the clinical case manager for additional resources and they will refer you for the next available appointment.
    If you have not been to CAPS or have not updated your paperwork for the current academic year during the fall semester, please arrive at your scheduled appointment 30 minutes early to fill out any paperwork.
